What accommodation documentation is, and how we build it

Formal accommodations — extended time on exams, a reduced course load, flexible deadlines, or workplace adjustments — almost always require current documentation from a qualified clinician. A self-report, an old school record, or a quick online quiz usually won't meet the standard that schools, disability-services offices, employers, and testing agencies apply.


A comprehensive evaluation closes that gap. Using a clinical interview, standardized measures, and a review of your history, your clinician confirms whether ADHD and/or autism is present and, just as importantly, documents how it affects you day to day. The written report ties those functional limitations to specific, reasonable accommodations — the evidence a decision-maker needs to say yes.

Is the evaluation done online, or do I need to come in person?

ScienceWorks conducts ADHD and autism evaluations entirely online by secure telehealth, for clients located in Tennessee during sessions. There's no in-person option — you complete the evaluation from home or work, and you receive your written documentation electronically.

Will the documentation be accepted for accommodations?

We prepare reports to meet the documentation standards schools, college disability-services offices, employers, and testing agencies commonly apply. Each organization makes its own decision, so we document your diagnosis and its functional impact to give your request the strongest support.

What does an evaluation cost, and will insurance help?

We review fees, scheduling, and any out-of-network reimbursement options before you commit, so there are no surprises. Many clients use FSA or HSA funds or monthly superbills, and we'll walk you through what applies to your situation.
